Vito Falcone, a crime family underboss, confines his nine-months-pregnant wife Alessia in a hidden medical facility and administers labor suppressants to delay her delivery. A blood oath with his deceased brother Daemon promises the lucrative West Coast territory to the firstborn grandson. Since Daemon's widow Scarlett is due simultaneously, Vito forces Alessia to endure agonizing induced delay so Scarlett's child can claim the birthright. When Alessia pleads for hospital care, Vito strangles her, falsely accusing her of manipulating Scarlett into signing a prenup that waived inheritance rights. He maintains a vigil at Scarlett's bedside until her son is born, only dispatching his lieutenant Marco to release Alessia afterward. Marco returns with devastating news: Alessia and their baby have died. The revelation shatters Vito completely.
